Family Peer Support Advocate

The Family Peer Support Advocate works in partnership with the program team and families with children experiencing social, emotional, developmental and/or behavioral challenges to facilitate communication and help ensure that the child’s interests are best represented in all settings.  This position requires some evening hours. Care Manager

The Care Manager is responsible for coordinating services and supports for individuals with a primary diagnosis of severe mental illness and/or other qualifying disorders in order to help successful integration into and maintenance in the community. Please see full job description for further details.


Outreach Specialist

The Outreach Specialist is responsible for promoting Health Home and Care Management services, and educating the surrounding resources, potential applicants and the community about these services. In promoting such services, the Outreach Specialist will spend some time in the community, with the expectation of generating appropriate referrals to the above programs, and ensuring that eligible applicants have access to these services.  Additionally, this position will provide intake assessments and provide services to engage Health Home clients identified as Outreach.  Please see full job description for further details.


Prevention Educator/Supervisor

The Prevention Educator is directly responsible for providing substance abuse and violence prevention education in assigned classes in school districts within Putnam County, while maintaining a positive relationship with school personnel.

Part-Time Opportunities


ChildCare Worker

This part time position is based out of our Carmel location.  This is a great opportunity for social work or child development college students.  We are seeking a responsible, reliable individual to work with our family empowerment program providing childcare for kids of parents attending meetings.  Meetings could be Monday, Tuesday or Thursday evenings with hours between 6:00pm and 8:00pm.

Quality Assurance Specialist-Chemical Dependency Clinic

The Quality Assurance (QA) Specialist is responsible for the design and administration of a comprehensive system that continually monitors and evaluates Chemical Dependency Clinic service delivery processes that are either directly, or indirectly, related to delivery of care and services.
